REINSTATEMENT:
The fee for reinstatement of a license shall be the sum of the license fees not paid for the preceding three years. The licensee will also be subject to a late renewal fine if the license is reinstated after July 1.
MAILING LIST:
The fee for a mailing list is fifty dollars ($50) per list.
INTERSTATE EXCHANGE OF INFORMATION:
The fee to complete an interstate exchange of information form is ten dollars ($10) per form.
